Most of the forms and brochures published on this site for you to download are in Adobe Acrobat Format (PDF).

 PDF is much like "electronic paper" in that it retains the original layout of the document. Formats like Microsoft Word will not retain the document's layout when opened on another user's computer.  As you would imagine, with something like a form, this is very important.

To get these forms there are two steps. First, if you don't already have it, you need to have the Adobe Acrobat Reader software to open, view and print the files you download from this site. This software is free, and may be downloaded from the Adobe Website by clicking on the "Get Acrobat Reader"  button.

Once you have downloaded and installed the Adobe Acrobat software, the next step is to download the file you want from the list below. 

Important: When selecting the file you wish to download, ensure you click with your RIGHT mouse button, then select "Save file as". If you left-click, all that will happen is your browser will attempt to open the file you have selected right away in your browser without downloading it, this can can be a slow process.
